Founding Full-Stack Engineer
About this role
We're looking for an exceptional full-stack engineer to join us as a founding member of the CareSwift engineering team. This is a rare opportunity to build healthcare technology from the ground up while having significant influence on technical architecture, product direction, and company culture.
As our Founding Full-Stack Engineer, you'll work directly with the CEO and existing technical team to build our AI-powered documentation platform that's already transforming how EMS providers capture patient care. You'll own entire features from database to UI, make critical technical decisions, and help establish engineering practices that will scale with our rapid growth.
Our Tech Stack: • Frontend: Next.js 14 with TypeScript, Tailwind CSS • Backend: Bun runtime with Hono framework • Infrastructure: Full Azure stack (HIPAA compliant) • Database: CitusDB (distributed PostgreSQL) • Caching: Redis • CDN/Security: Azure Front Door • AI/ML: OpenAI APIs, custom models

What you'll do
- Architect and implement scalable features across our entire stack, from database schema to React components
- Build HIPAA-compliant APIs and services using Bun and Hono that handle sensitive healthcare data
- Design real-time data synchronization systems for offline-capable mobile applications
- Implement AI-powered features that help EMS providers document patient care more efficiently
- Collaborate on technical architecture decisions that will impact the platform for years to come
- Establish engineering best practices including code review processes, testing strategies, and deployment pipelines
- Work directly with EMS providers to understand workflows and translate them into technical solutions
- Optimize performance across our distributed PostgreSQL (CitusDB) deployment
- Build monitoring and observability into our Azure infrastructure from day one
- Mentor future engineers as we grow the team
What we're looking for
- 5+ years of professional software engineering experience with full-stack development
- Expert-level TypeScript skills and deep understanding of modern JavaScript
- Strong experience with React/Next.js and building performant, accessible web applications
- Proven track record building scalable backend services and APIs
- Experience with distributed systems and database scaling (PostgreSQL experience preferred)
- Comfortable with cloud infrastructure, particularly Azure or equivalent platforms
- Understanding of security best practices and compliance requirements (HIPAA experience a plus)
- Excellent communication skills and ability to work effectively in a remote environment
- Self-directed with strong ownership mentality - you see problems and fix them
- Genuine interest in healthcare technology and improving emergency medical services
Nice to have
- Experience with Bun runtime or similar modern JavaScript runtimes
- Familiarity with Hono or similar lightweight web frameworks
- Previous experience in healthcare technology or with HIPAA compliance
- Knowledge of Redis caching strategies and implementation
- Experience with AI/ML integration in production applications
- Background in building offline-first or real-time collaborative applications
- Experience with Azure services (Functions, Service Bus, Cosmos DB, etc.)
- Contributions to open source projects
- Previous startup or founding engineer experience
